Milwaukee's Unified Emergency Operations Center (UEOC), made up of the 19 municipalities in Milwaukee County along with the public health officers, health care providers and public safety officers within the County, formed in March of 2020. The Mission of the UEOC effort is to coordinate and elevate a unified multi-sector public health response to COVID-19’s direct and indirect effects, with emphasis on affected vulnerable populations – in collaboration with key sectors in Milwaukee County.
The UEOC has a COVID-19 Vaccine Coordinating Committee and a number of subgroups. UEOC has stood-up a “clearinghouse” to facilitate matching unaffiliated 1a organizations and/or individuals with COVID-19 vaccine. Individuals and organization can sign-up by visiting HealthyMke. UEOC is requesting that anyone in Milwaukee County use this platform so, they can have a coordinated effort to provide all of Milwaukee County’s phase 1a providers with vaccine. It is important to understand that vaccine supply remains limited at this time. UEOC asks for patience as they work to get everyone matched.
City of West Allis
At this time the West Allis Health Department is working to connect with unaffiliated health care personnel who qualify for Phase 1a of the COVID-19 vaccine.
There is vaccine information on the city of West Allis website. Unaffiliated health care personnel can fill out this form to express interest in receiving the vaccine. Please contact ksutton@westalliswi.gov with member information or other questions.

The Washington Ozaukee Public Health Department is registered as a Vaccinator with DHS. The health department is prepared to vaccinate health care providers within the 1a designation who do not have access to the vaccine through their own provider or a health care system. It is unclear if and when the health department will receive vaccine or how many doses will be delivered. When the health department does receive vaccine, we want to assure we are prepared to distribute it to individuals within the 1a designation.
In order to ensure Ozaukee Washington County has the information needed, please complete a brief survey as soon as possible.
This request is strictly for individuals within the 1a designation (dentists, dental hygienists/assistants, behavioral health practitioners, chiropractors, home health services/hospice, independent therapists (physical, speech, occupational), school nurses/nurse aides, and transportation services). We recognize we do not have an exhaustive list of contact information, so please feel free to share with your professional colleagues.
Once vaccine is available, you will likely receive a time sensitive email asking your staff to report to our vaccination clinic within 24-48 hours. Agencies contacted to receive vaccine will have one opportunity to respond. We will vaccinate individuals who fall into 1a who live and/or work in our counties.

Independent health care workers are asked to call Pierce County Public Health Department’s health care worker vaccination direct line at 715.307.9575. Pierce County will then verify your license or credential and gather your contact information.
If a health care worker is employed as part of a health care system, clinic, pharmacy, or dental office, we ask that they speak with their employer about the plan to receive vaccine.
Any unaffiliated 1a workers in Polk County should contact the Public Health Department at 715.485.8400 to have their name added to the list. They will be notified when and where a vaccine is available and the next steps to schedule.

Central Racine County Health Department (CRCHD) requests Racine County independent physicians fill out a Phase 1A survey if they have not already found a vaccination site.
CRCHD will follow up to collect names and contact information of individuals eligible for the vaccine. Individuals will be matched with appointments as the vaccine becomes available.
